Connected Care Ecosystems
The concept of remote care has demonstrated that care can be delivered from home or distant settings. The larger implementation challenge in remote care is turning the digital initiatives in silos into a connected model that can operate consistently across patients, providers, and care settings. The enablement requires understanding the needs & constraints of multiple stakeholders, including patients, HCPs, clinic administrators, payors, regulators, and government.
Technologies such as cloud-based platforms, AI-enabled population-health tools, and digital workflows enable interactions across the patient journey. Other enabling technologies like 5G-enabled remote surgical assistance and point-of-care robotics are expanding the coverage. At the same time, interoperability is becoming a fundamental requirement. With new mandates establishing new standards for health-data exchange, they are increasingly influencing how connected-care platforms are designed and integrated.
However, technology alone cannot drive connected care adoption. Our assessments show that successful implementation of connected care starts with building an implementable care scope, governance policies, operational processes, and pricing models.
Many virtual-care programs lose momentum after the pilot stage because reimbursement models, EHR integration, and clinician workflows are not being addressed alongside the technology. Fragmented platforms can further add complexity rather than improving coordination if they are not designed around existing clinical processes.
Stellarix helps health systems, payers, and digital-health companies build connected-care models around the realities of clinical operations. Our work spans virtual-care strategy, digital-platform roadmaps, care coordination, operating-model design, interoperability, and emerging-technology assessment. We have helped organizations address the integration, workflow, and operating-model issues that determine whether digital-care initiatives can scale.
Our Experience:
- Developed a framework & policy document for consolidating multiple virtual-care initiatives for a regional health system, identifying reimbursement, EHR integration, and workflow gaps and developing a phased roadmap toward a common care-platform model.
- Evaluated and compared capabilities, integration requirements, and clinical workflow of 30+ population-health platforms and solutions for a digital-health provider, to identify the areas with the strongest market potential.
- Evaluated 15+ 5G-enabled remote surgery and point-of-care robotics platforms by assessing their technology readiness, functional benefits to HCPs, & interoperability to shortlist and evaluate partnership potential suiting our clients’ requirements.
