Decentralized Care Delivery
Care is moving beyond the hospital to homes & remote settings. But the operating models, governance, and risk frameworks required to support that shift are not evolving at the same pace.
Technologies like remote patient monitoring, telehealth, hospital-at-home services, wearables, and mHealth are pushing the range of care delivery outside traditional clinical settings. AI-enabled monitoring supports earlier intervention, while hospital-at-home models are beginning to bring more complex services into patients’ homes. This shift can improve access and convenience while creating new opportunities to manage care around the patient rather than the facility.
Scaling these models, however, goes beyond clinical validation or technology deployment.
Our assessments demonstrate that most remote-monitoring and hospital-at-home programs face barriers around governance models, people’s misunderstanding of the job role, clinical liability, reimbursement, and health-equity considerations, rather than just technology performance.
In most cases, pilot projects lost momentum because workflows didn’t sync with decentralized delivery or home-based care delivery. Expanding access without addressing issues of various stakeholders, including HCPs, patients, administrators, investors and payors, would create operational risks, and the intended clinical benefits would not be achieved.
Providers and payers therefore need to determine which services can and cannot be safely decentralized, what governance is required, and how reimbursement and workflows need to change. Patient equity also needs to be considered, particularly when digital models are intended to reach underserved and bottom-of-the-pyramid populations.
Stellarix helps providers, payers, and digital-health companies evaluate success stories, build frameworks, and develop scalable decentralized-care models. Our experience spans remote-monitoring strategy, telehealth expansion, hospital-at-home design, adoption-risk assessment, and wearable and mHealth technology evaluation. We are helping clients address the risks and operational barriers that prevent decentralized-care programs from moving beyond the pilot stage.
Our Experience:
- For governance framework development, Stellarix assessed the success stories, clinical challenges, reimbursement, and patient-adoption barriers affecting an AI-enabled remote-monitoring program. Further, built a risk-mitigated roadmap for our client to move from pilot to broader deployment.
- Developed a clinical, operational, and governance framework for an advanced hospital-at-home service, translating the client’s strategic ambitions into a scalable care-delivery model.
